Pay Advance Consent

Family First Caregivers:

As part of our payroll conversion to ADP’s enhanced platform, we are changing the Family First pay cycle to match the same pay cycle as the rest of HomeCentris’ caregivers.  This new pay cycle will be Monday through Sunday.  To make this change, the pay week that starts on Thursday, December 25 will end on Sunday, December 28, instead of Wednesday, December 31.  This means that your final 2025 pay period will be a four-day work week, not a regular seven-day work week.  Because of this one-time change, your 12/31/25 paycheck will be three days shorter than normal. This is a one-time adjustment, and future paychecks will again be seven days.  To help with this transition, HomeCentris is offering an optional pay advance for the three fewer days of pay (based on your scheduled hours).  If you choose the advance, it will be paid to you in your December 31 check and then repaid in three equal deductions over your next three paychecks after the new pay cycle begins.  Please choose which option you prefer.

NOTE:  If you do not submit your preference, your 12/31/25 paycheck will reflect the four days from 12/25 to 12/28.

Yes, I agree to receive an advance for the three transitional days of pay based on my scheduled hours for those days. I understand and consent that one-third of the advance will be deducted from each of my next three paychecks. I agree that if I receive the advance and leave HomeCentris before it is fully repaid, any remaining balance will be deducted from my final paycheck, as allowed by law.

No, I do not want the advance. I understand that my 12/31/25 paycheck will reflect a four-day work week, and I am comfortable moving to the new pay cycle without receiving an advance.
