HomeCentris Home Health provides short-term skilled nursing and therapy services. Our patients usually need these services after a hospitalization or a discharge from a nursing home or rehab center. Home health services are ordered by your doctor and typically last for 30 to 60 days. We also provide pre-acute preventative services to help manage medications or chronic conditions. Our home health services are 100% reimbursed for eligible Medicare recipients and we also accept commercial insurance.
Our Skilled Home Health agencies, in Maryland and Virginia, are accredited by CHAP. Accreditation is a process of third-party review that allows healthcare organizations to demonstrate their ability to meet regulatory requirements and standards established by a recognized accreditation organization. Skilled Nursing
Skilled nursing can help after injury, illness, or surgery or, for care needs related to a chronic condition. We can provide services such as complex wound dressings, medication management, disease management, patient monitoring, and education to help you recover as quickly as possible. We also offer skilled nursing services to help you manage long-term, chronic conditions so you can do more of what you like to do.
Physical Therapy
Physical therapy can help improve your physical function through an exercise program, balance training, and education. Our Physical Therapists will help you regain your strength, endurance, and flexibility.
Occupational Therapy
For improving motor skills and reasoning abilities, or to compensate for permanent loss of function. Our Occupational Therapists help our patients with their ability to do everyday activities safely in the comfort of their homes. We may assess and modify our patients’ home and community environments to improve their safety and independence and we may prescribe and educate patients and caregivers in the use of adaptive equipment to assist function.
Speech Therapy
Speech therapy is used for evaluating and treating swallowing, cognition, and communication problems after a stroke or other neurological condition. Problems could include stuttering, apraxia, dysarthria or language disorders such as aphasia. Our speech therapists can provide oral motor exercises to help our patients improve their speaking ability and help them communicate more clearly.

To provide assessments and connect you and your loved ones to community resources to help manage your condition. Further, we offer a variety of specialty programs aimed at special conditions. Each contains specific protocols designed to speed your recovery and get you into the highest activity level possible.
